Advanced Practice Provider - Advanced Lung Disease- COPD

The Advanced Practice Provider (APP) provides medical services to individuals, families and groups, emphasizing
health promotion and disease prevention. This includes the assessment, diagnosis and management of acute and
chronic diseases. Responsible for performing job duties in accordance with mission, vision and values of TampaGeneral Hospital.

Job Description:

Provides longitudinal outpatient management of patients with COPD, including new and follow-up visits, post-hospital care, medication and inhaler optimization, oxygen management, review of diagnostic testing, patient education, and coordination with pulmonary rehabilitation and other services. The role is designed to improve access to pulmonary care, optimize COPD management, identify clinical deterioration early, and reduce preventable exacerbations, ED utilization, and hospital readmissions.
