HOURS: Full-time, 40hours, Monday - Friday.
SALARY: Non-Exempt. Dependent on experience and qualifications.

A Day in the Life
No two days are the same as a Re-Entry Coach. You may start your morning meeting with clients, reviewing goals, addressing barriers, and celebrating successes. Throughout the day, you'll connect clients with housing resources, employment opportunities, treatment services, and community supports. You'll collaborate with supervising officers and community partners to ensure clients stay on track and have access to the resources they need. Between appointments, you'll document client progress, update case plans, and maintain accurate records. Success in this role requires strong boundaries, professionalism, reliability, attention to detail, and a genuine desire to help others achieve positive change.
RESPONSIBILITIES:
- Assess client needs and develop individualized case plans.
- Provide ongoing coaching and support in areas such as housing, employment, transportation, budgeting, and life skills.
- Coordinate and connect clients to community resources and supportive services.
- Maintain regular contact with clients and their supervising officers.
- Facilitate evidence-based programming and skill-building activities.
- Collaborate with community partners and participate in outreach efforts.
- Maintain accurate, timely, and detailed documentation in required databases and systems.
- Track outcomes and ensure compliance with program requirements.
- Uphold professional boundaries and ethical standards when working with clients.
- Represent LSS in a positive and professional manner within the community.
QUALIFICATIONS:
- Bachelor’s degree, plus one year experience working in human service environment; or equivalent combination of higher education and experience, is required.
- Preferred experience working with justice involved individuals, trauma informed care and adult case management.
- Must be able to complete orientation training according to agency program standards.
- Be at least 21 years old and willing to complete agency background checks.
- Maintain a valid driver’s license or obtain one with approved driver status.
- Must successfully complete all agency background checks, which include checks to detect criminal convictions of behavior harmful towards children, spousal abuse, and drug or sex crimes.
