About the role
Primary Care Float Provider | Central City Concern
Float where you’re needed most - All of the care. None of the panel.
If you love the variety and impact of frontline primary care but not the weight of a full patient panel, this role was built for you. As a Float Provider, you won’t carry a panel. You’ll be the provider our clinics count on, moving where the need is greatest and bringing steady, skilled care to every site.
Home bases
- Old Town Clinic Walk-In Clinic (WIC): urgent care, wound care, and addiction services
- Blackburn Center: a full range of primary care services
Float to
- Evergreen / Recuperative Care Program: medical respite for people recovering from illness or injury
- Hansen Clinic: care delivered inside a shelter, right where people are
- Mobile Health: care that goes out to meet people in the community
One week you might be treating an acute illness, the next you’re managing wounds, supporting someone’s recovery, or providing care in a shelter. No two days look the same.
Why Providers love this role
- No panel, no backlog. Spend your energy on the patient in front of you.
- Broad, real-world scope. Urgent care, wound care, addiction medicine, chronic disease management, and procedures across multiple care settings.
- Meaningful work. Care for neighbors who are too often overlooked, using shared decision-making, motivational interviewing, and trauma-informed, non-judgmental practice.
- Integrated services. Coordinate with Hepatitis C treatment, withdrawal management, residential substance use treatment, specialty mental health, and overdose prevention programs.
- Room to grow and lead. Shape clinical workflows, contribute to quality improvement, and help teach and onboard fellow providers and staff.
You might be our next float provider if you...
- Have primary care or mobile medical experience
- Are adaptable, steady, and comfortable stepping into a new setting and building quick rapport
- Practice with a non-judgmental, trauma-informed, equity-minded approach
- Know de-escalation methods, or are willing to learn them
- Enjoy independent practice backed by a strong team
- Believe everyone deserves excellent care, no matter their circumstances
