· Coordinate new member referrals, admissions, and timely service initiation following required approvals.
· Work with HR and the RN to confirm caregiver clearance, coordinate member-specific training, and verify readiness before the first scheduled visit.
· Maintain communication with members, families, caregivers, and care coordination teams.
· Respond promptly to staffing challenges, missed visits, and service interruptions.
Virginia Medicaid Program Coordination
· Coordinate agency services in accordance with applicable Virginia Medicaid, Department of Medical Assistance Services (DMAS), and managed care organization requirements.
· Review service authorizations, approved hours, and applicable service plans.
· Communicate with Medicaid care coordinators, health plan representatives, and referral partners.
· Monitor service utilization and follow up on authorization renewals, discrepancies, and potential service interruptions.
· Maintain accurate documentation and follow through on referrals and ongoing service needs.
· Coordinate with the RN and appropriate internal teams on care plans, supervision needs, and changes in member needs.
Operations & Compliance
· Monitor electronic visit verification (EVV), visit documentation, and scheduling accuracy.
· Identify and help resolve visit exceptions, documentation discrepancies, and service-related concerns.
· Incident reporting, quality assurance, and operational follow-up.
· Track unstaffed hours, caregiver availability, and outstanding service needs.
· Support compliance with applicable Virginia Medicaid provider requirements and company policies.
· Use home care management software to maintain accurate records and coordinate with HR, clinical staff, billing, and leadership.
Program Growth & Community Relationships
· Build positive relationships with Medicaid care coordinators, referral sources, and community partners.
· Conduct outreach and follow up consistently on referral opportunities.
· Support member referrals, caregiver recruitment, and growth within the agency’s approved service areas.
