About the role
Licensed Practical Nurse (LPN) – PACE
Schedule: Monday-Friday, 8:00am-5:00pm
Join HopeWest and be part of a team that is changing the way healthcare supports aging adults in our community.
At HopeWest, we believe exceptional care begins with exceptional people. We are looking for a compassionate, skilled Licensed Practical Nurse (LPN) to join our growing PACE (Program of All-Inclusive Care for the Elderly) team.
The Licensed Practical Nurse (LPN) provides direct participant care, supports providers during examinations and procedures, and assists with care coordination in a clinic setting. This role ensures efficient patient flow, delivers high-quality nursing care within scope of practice, and supports clinical operations to promote positive participant outcomes.
Why PACE?
PACE is more than healthcare—it is a different way of caring for older adults.
Instead of focusing on one appointment or one health concern at a time, our interdisciplinary team works together to support the whole person. This collaborative approach allows you to build meaningful relationships with participants while working alongside professionals who share a common goal: helping people live with dignity, independence, and quality of life. PACE provides comprehensive, coordinated healthcare and supportive services that allow older adults with complex medical needs to remain living independently in their homes and communities for as long as possible. As an LPN, you will play an important role in delivering hands-on care, supporting participants and families, and collaborating with an interdisciplinary team committed to providing person-centered care.
What You Bring
Current and valid Licensed Practical Nurse (LPN) license in the State of Colorado, maintained throughout employment.
Graduate of an accredited practical nursing program.
Current and valid Colorado driver's license, adequate automobile insurance and unlimited access to reliable transportation.
1-2 years experience as an LPN, experience working in a clinic, outpatient or hospital setting preferred.
Strong clinical judgment and the ability to recognize and communicate changes in participant condition.
Excellent communication and interpersonal skills.
A collaborative, flexible approach to patient care.
A genuine commitment to compassionate, person-centered care.
