About the role
Description
Be the Difference Between Crisis and Recovery
At Concord Counseling Services, we believe that people experiencing serious mental health challenges deserve more than crisis response. They deserve hope, stability, support, and someone willing to walk alongside them through some of the most difficult moments of their lives.
We are seeking a compassionate, resilient, and highly motivated Crisis Case Manager to provide intensive community-based services to individuals experiencing chronic or recurring behavioral health crises. In this role, you will work directly with adults facing serious mental illness, psychiatric hospitalization, housing instability, substance use concerns, and other significant life challenges.
As a Crisis Case Manager, you'll help individuals transition successfully from hospitals back into their communities, connect them to critical services, strengthen their support systems, and reduce the need for future crisis interventions. Every day brings a new opportunity to help someone move from instability to recovery and from surviving to thriving.
What You'll Do
•Build trusting relationships with individuals experiencing complex behavioral health challenges.
•Provide intensive, person-centered case management services in community settings, homes, hospitals, and other natural environments.
•Assist clients in navigating behavioral health treatment, medical care, housing, employment, public benefits, and community resources.
•Support individuals during times of crisis and help them develop strategies that promote long-term stability.
•Participate in hospital treatment team meetings and discharge planning to ensure a successful return to the community.
•Coordinate care among clients, families, healthcare providers, social services, and community partners.
•Advocate for clients while helping them develop the skills and confidence necessary to live as independently as possible.
•Monitor outcomes and support progress toward goals that improve functioning, reduce hospitalizations, and enhance quality of life.
Requirements
Qualifications
•Bachelor's degree in Social Work, Counseling, Human Services, Psychology, or a related field required.
•Ohio licensure as an LSW, LPC, or other qualifying behavioral health credential preferred.
•At least one year of experience providing crisis services, community psychiatric support, intensive case management, or related behavioral health services preferred.
•Knowledge of serious mental illness, co-occurring disorders, crisis intervention, and community resources.
•Valid driver's license, reliable transportation, and the ability to travel throughout the community.
