Position Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide skilled physical therapy interventions to improve strength, mobility, balance, endurance, and functional independence.
- Monitor patient progress and adjust treatment plans as needed.
- Document evaluations, treatment sessions, progress notes, and discharge recommendations accurately.
- Collaborate with nursing staff, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team.
- Educate patients, families, and caregivers on exercises, safety, and functional mobility.
- Maintain compliance with applicable clinical, regulatory, and documentation requirements.
Qualifications
- Current, unrestricted Physical Therapist (PT) license in the state of Michigan.
- Graduate of an accredited Physical Therapy program.
- Strong communication, organizational, and clinical skills.
- Ability to work effectively with patients, families, and an interdisciplinary team.
- Skilled nursing facility (SNF) or geriatric rehabilitation experience preferred.
